As a Community Indigenous Patient Navigator, you will provide guidance to Indigenous clients and families within Horizon’s healthcare system. This permanent, full-time position requires robust mediation skills, knowledge of Indigenous healing practices, and the ability to foster trusting relationships. Your dedication will enhance the overall health of clients while advocating for culturally appropriate healthcare access.
Key Responsibilities:
• Support Indigenous clients in community health settings
• Facilitate access to cultural, spiritual, and linguistic resources
• Conduct health literacy assistance and empower client involvement
• Collaborate with Indigenous families and healthcare teams
• Develop educational materials on Indigenous health
Requirements:
• 1-3 years’ experience in Indigenous health
• Strong understanding of cultural safety
• Proficiency in Microsoft Office applications
• Education in health or social sciences
• Must be of Indigenous descent
