This role requires a degree in a Health Care Discipline, with a Master’s level preferred. You will engage in consultation, education, and capacity-building initiatives while working with various healthcare providers to improve mental health outcomes for seniors.
Key Responsibilities:
• Provide case-based consultations to healthcare providers
• Develop relationships with community healthcare partners




• Assist in identifying community learning needs
• Collaborate as part of the SMH BSS team
• Support effective service delivery models for seniors
Requirements:
• Degree in Health Care Discipline; Master’s preferred
• Minimum three years in community healthcare
• Expertise in seniors mental health assessment
• Knowledge of PIECES framework and GPA principles
