Key Responsibilities:
• Provide comprehensive primary health care services
• Develop individualized care plans and health strategies
• Deliver patient-centered, culturally safe care
• Collaborate with healthcare providers for coordinated care
• Participate in community outreach and health promotion
Requirements:
• Current registration with the College of Nurses of Ontario
• Completion of a recognized Nurse Practitioner program
• Experience in primary care preferred
• Understanding of Indigenous health and cultures
• Excellent clinical and communication skills
