As a permanent full-time role, the Integrated Care Manager will oversee integrated care delivery, managing interprofessional teams while promoting collaborative practices. This position focuses on ensuring high-quality service across clinical, wellness, and health education sectors, directly contributing to robust population health outcome objectives. The Manager will report to the VP, Integrated Care and Experience and will be pivotal in operationalizing DCHC's strategic priorities.

Key Responsibilities:
• Implement strategic and operational plans across programs
• Supervise and mentor interprofessional staff teams




• Monitor budget and program performance against targets
• Champion quality improvement initiatives
• Support recruitment, onboarding, and staff development

Requirements:
• Undergraduate degree in health care or related field
• 5 years in community-based health or social services
• Experience in integrated care and team management
• Strong communication and leadership skills
• Familiarity with Ontario's health systems and performance frameworks
