UHN is hiring three full time pharmacists for the Pharmacy Department at Toronto Western Hospital. You will conduct medication histories, resolve drug therapy issues, and monitor patient drug therapies. Ensuring medication safety and providing patient education are key aspects of this role.
Key Responsibilities:
• Conduct best possible medication history interviews
• Review medication orders for appropriateness
• Monitor drug therapy for effectiveness and safety
• Educate patients on medication use and safety
• Supervise and support pharmacy technician staff
Requirements:
• Bachelor's or Doctor of Pharmacy degree required
• Active registration with Ontario College of Pharmacists
• Hospital pharmacy practice experience is essential
• Excellent communication and customer service skills
• Organized with strong time management abilities
