This role provides support, education, referrals, and coordination of care to patients and their families prior to, during, and following their healthcare experience. Working closely alongside nursing staff and APPs, the Navigator facilitates integrated care through communication with the interdisciplinary team, collaborates on staff education, and promotes evidence-based clinical practices that exemplify the pursuit of clinical excellence. We are looking for a self-starter who is passionate about adult cardiology and committed to optimizing patient satisfaction and achieving the best possible outcomes.Your Role Within The TeamServe as the primary liaison between inpatient and outpatient care teams for adults with congenital heart defectsCoordinate efforts between physicians,
