**Position Summary**
This is an exciting opportunity for an cutting-edge and experienced Registered Nurse (RN) to join the Family Navigation Hub (FNH) team.

The RN in this role will be part of establishing a nursing role in the FNH team.

The Ambulatory Care Nurse (ACN) - Registered Nurse provides leadership and promotes excellence in pediatric, family-centred rehabilitation nursing care.

The paediatric rehabilitation RN skill set is grounded in rehabilitative principles of managing complex medical needs, collaborating with the interprofessional team, partnering with the client and family, providing client/caregiver education, working with the client/family to set goals for maximum independence and establish plans of care to maintain optimal wellness and coordination of services.

The skill set is grounded in rehabilitative principles of assessing, managing and evaluating complex health conditions in collaboration with the interprofessional team and client/family.

They collaborate with the client and family, to establish plans of care to obtain optimal health and wellbeing, utilizing a strength-based approach.

As a member of the

Family Navigation Hub (https://hollandbloorview.ca/services/programs-services/family-navigation-hub) team, the ACN has knowledge and experience working with children and youth with disabilities and developmental differences and their families, including clients and families who identify as newcomers.

They provide proactive, upfront supports to clients and families while they await their first appointment, partnering to identify and address unmet social needs.

They participate in educational activities, program committees,



quality improvement projects or other operational support projects as assigned and provide nursing services to clients and families based on standards of practice set out by the College of Nurses of Ontario.

The ACN provides leadership and promotes excellence in pediatric family centred care within an interprofessional, academic and collaborative practice environment.

The ACN is registered with College of Nurses of Ontario and works to full scope of practice within the current legislative framework.

The RN demonstrates the competencies to work across various ambulatory care settings as required.
**Key Responsibilities**
**Clinical Practice**
Uses evidence-based practice to plan, implement and evaluate nursing care for clients within the Family Navigation Hub and various ambulatory clinics.

Provides nursing care utilizing a strengths-based approach that promotes and supports the physical, psychosocial, and spiritual well-being of clients and families.

Provide education and support to patients and their families about treatment plans and lifestyle modifications during and between clinic visits.

Practices culturally competent care.

Monitor client's progress and provide support to the client and family in implementing recommendations.

Serve as a point of contact for patients and families, coordinating with other team members and ensuring seamless care delivery.

Initiates and coordinates referrals and support with system navigation with regards to funding, community resources and other healthcare services.

Knowledge of social determinants of health and equity-oriented care practices in connecting families to practical supports (e.g. food, housing, financial resources).

Collaborates with clients/families, members of the interprofessional team and community health care providers to ensure an optimum and integrated approach to care.

Bases practice on nursing science and content relevant to nursing from the sciences and humanities and established professional practice standards.

Supports the maintenance of a safe and healthy work environment and advances a culture of client/patient safety through work and daily practices.
**Collaboration**
Collaborates with client/family and interprofessional team to identify client-centred goals.

Demonstrates excellent communication skills.

Develops and maintains partnerships with community providers.
**Professional Development/Leadership**
Contributes to the development of professional nursing practice through participation and leadership in the Family Navigation Hub.

Demonstrates a commitment to lifelong learning and continuous development of self by identifying learning needs and establishing annual goals and objectives.

Maintains own knowledge, skills, and abilities through education, and training both in-house and in post-secondary institutions.

Manages change in an inquisitive and client centered manner.





Ensures that nursing practice issues are addressed using appropriate lines of communication within the organization.

In partnership with clients/families and the interdisciplinary team, provides client and family-centred care which is timely, goal directed, evidence based and outcome oriented.

Supports and respects the client/family's rights to make decisions based on their own values, beliefs and experiences.
**Research**
Demonstrates and utilizes the teaching/learning process to provide education, both formal and informal to clients/families, colleagues, students and other professionals within a fully affiliated academic healthcare environment.

Shares new knowledge and best practices with colleagues and other professionals within the organization and the community.

Demonstrates a spirit of inquiry by examining current practice and identifying questions for study.

Participates in quality initiatives and utilizes outcome measurements.
**Education**
Currently registered as an RN with the College of Nurses of Ontario.
**Experience**
Minimum of one year of nursing experience in a pediatric environment preferred.

Experience, knowledge or interest in a rehabilitation setting and/or ambulatory care nursing role.
**Certification/Licenses**
Current certificate of competence by the College of Nurses of Ontario.

Current BCLS certificate—Basic Rescuer.

Certification in Pediatric Nursing from Canadian Nurses Association is preferred.

Membership in RNAO preferred.
