**Position Summary**
As a member of the

Family Navigation Hub (https://hollandbloorview.ca/services/programs-services/family-navigation-hub) team, the Social Worker will provide support to children and youth with disabilities and developmental differences and their families, including clients and families who identify as newcomers.

The Social Worker will provide proactive, upfront supports to families while they await their first appointment, partnering to identify and address unmet social needs.

This is a fast paced, dynamic work environment requiring the candidate to work independently and collaboratively with other professionals and outpatient teams involved in the client and family's care.
**Key Responsibilities**
Conducts strengths-based Social Work assessments for clients and families of diverse backgrounds who are preparing for their first clinical appointment.

In collaboration with the interdisciplinary team,



support clients and families to enhance coping and advocacy skills.

Identifies need for social work intervention through case screening and case finding methods.

Supports and respects the client/family's rights to make decisions based on their own values, beliefs and experiences.

Provides resource counselling to clients utilizing evidence-based techniques based on client's presenting mental health concerns
Initiates and coordinates referrals and support with system navigation with regards to funding, community resources and other healthcare services
Knowledge of social determinants of health and equity-oriented care practices in connecting families to practical supports (e.g., food, housing, financial resources).

Advocates for client and family needs for service within Holland Bloorview and the community.

Bases practice on established Holland Bloorview policies, procedures, evidence, and best practice.

Documents accurate workload statistics.

Participates in quality improvement initiatives, contributes to the academic agenda through education or research, and supports the maintenance of a safe and healthy work environment that advances a culture of client/patient safety through work and daily practices.

Participates in the academic agenda through the supervision of students.

Adheres to Holland Bloorview's Documentation Policy
Communicates in a timely and effective manner
Contributes to the development of program/service activities
Ensures the maintenance of the OCSWSSW Standards of Practice and Code of Ethics for Social Workers.

Remains current with professional literature and best practice guidelines.

Shares current knowledge and best practices with colleagues, staff, students and the community.
**Qualifications**
MSW Degree from an accredited University, Faculty of Social Work.

Current member of the Ontario College of Social Workers and Social Service Workers.

Current member of the Ontario Association of Social Workers preferred.

Paediatric Social Work or health care experience preferred.





Previous experience working with youth with disabilities and their families as well as an understanding of transition related concerns is an asset.

Demonstrated knowledge with solution focused coaching and/or motivational interviewing and an ability to apply them in session, as appropriate.

Experience working in an interdisciplinary setting preferred.

Works effectively as a member of the interdisciplinary team.

Knowledge of social determinants of health and equity-oriented care practices in connecting families to practical supports (e.g., food, housing, financial resources).

Demonstrates ability to manage and respond to crisis calmly and effectively.

Demonstrates ability to organize time and meet deadlines.

Demonstrates initiative and self direction.
