We currently have two contract Business analyst/Project management roles available for a 3-month period. Our client needs some extra help with phase 2&3 of a regional system improvement project.

The project has two objectives:

- Ensuring there is an agreed model for coordination
- Identifying, designing, documenting and implementing improvements

Additional resource is needed to support with Phase 2 and Phase 3, which will include work in the following areas:

Phase 2: Discovery

Use process improvement methodologies and team-oriented approaches in partnership with key stakeholders and responsible parties to:

- Discover, document and understand existing coordination and management processes, roles, responsibilities
- Explore examples of best practices in place in other regions
- Understand contracting, commissioning and data sharing enablers and constraints
- Understand working relationships.
- Document current state coordination model at regional and district/local levels i.e. process maps/visual workflows
- Document current state of processes at regional and district/local levels that enable current state coordination model i.e. process maps/visual workflows
- Identify inconsistencies, gaps and areas for improvement to align with national policies and expectations




- Identify inconsistencies, gaps and differences in approach and areas for improvement at district/local levels
- Provide a summary of findings and next steps for leadership.

Phase 3: Systems Improvement Design

- Identify and design system and process improvements to be included in:
- Redesigned/future state coordination model at regional and district/local level
- Redesigned/future state regional and district/local process
- Including consistent regional processes and quality assurance framework, consistent coordination process for allocation of referred individuals, overdue individuals and at-risk individuals and consistent processes for providers for case managing
- Seek formal agreement and approval of identified systems and process improvements
- Plan implementation phase including a Continuous Improvement approach that can be delivered as BAU regularly.
