Summary of Duties

To provide a full range of social work services to General Internal Medicine patients and their families on the medical units. Services include psychosocial assessments, counselling, discharge planning, linkage to community resources, facilitating Advance Care Planning Conversations and addressing complex care issues.

The successful candidate will participate as a key member of the patient care team on General Internal Medicine.

Qualifications/Skills
Master's Degree in Social Work required
Member in positive standing of the Ontario College of SW and SSW
Two years experience working as an MSW in a Hospital setting, acute medicine preferred
Experience with Gerontological patients an asset

Knowledge of the Health Care Legislation, health care system and community resources
