You have expert clinical skills in the assessment, planning, implementing, evaluating, and documenting the delivery and coordination of care for the CHF patients and their families. You have excellent communication and interpersonal skills. You have excellent leadership, collaboration, decision making, problem recognition, and problem-solving skills. You have excellent change management skills and also project management skills. You have evidence of program design and resources development skills. You also have excellent practice program development, project management,



and report writing skills. You have a high level of initiative, work independently, and can effectively lead project teams to achieve outcomes. This position supports the hospital’s academic mandate by advancing care through teaching and learning and/or transforming care through research and innovation.
