Your expertise will be vital in dealing with challenges related to cognitive impairments, ensuring optimal patient support. Key Responsibilities:
- Conduct behavioural assessments as per best practice guidelines
- Collaborate with caregivers to identify behaviour triggers
- Implement and document tailored behavioural interventions
- Provide training to staff involved in patient care
- Develop individualized transitional care plans Requirements:
- Community college diploma in healthcare, Bachelor's preferred
- One year experience with seniors in acute settings
- Certified in gerontology preferred
- Robust communication and interpersonal skills
