Overview
HarmonyCrew Home Health Care Inc. is a growing home care agency serving Surrey and surrounding communities. We provide compassionate, non-medical home support services that help seniors and adults remain safe, comfortable, and independent in their own homes. We are seeking an experienced Care Coordinator / Business Development Coordinator who is passionate about delivering exceptional client care while helping grow a new home care organization.
This is an exciting opportunity to become one of our first key team members and play an important role in building client relationships, supporting caregivers, and contributing to the growth and success of HarmonyCrew.
Duties
- Conduct client consultations and in-home assessments to determine individual care needs.
- Develop and maintain individualized care plans.
- Coordinate and monitor high-quality home support services.
- Match caregivers to clients based on care needs, personality, availability, and experience.
- Recruit, interview, onboard, and support caregivers.
- Coordinate caregiver schedules and respond promptly to scheduling changes, urgent staffing needs, and service requests.
- Serve as the primary point of contact for clients, families, caregivers, and referral partners.
- Build professional relationships with community organizations, senior-serving professionals, retirement communities, assisted living facilities, healthcare providers, physiotherapy clinics, occupational therapists, social workers, case managers, and other referral partners to generate client referrals.
- Promote HarmonyCrew Home Health Care services through networking, community outreach, presentations,
and relationship-building activities.
- Perform supervisory visits, quality assurance reviews, and client satisfaction follow-ups.
- Maintain accurate client, caregiver, scheduling, and operational records using office software or CRM systems.
- Prepare client service agreements, care documentation, and other administrative records.
- Support compliance with company policies, privacy requirements, WorkSafeBC procedures, and applicable standards.
- Assist with hiring events, caregiver orientation, training coordination, and ongoing employee support.
- Collaborate with management to improve operational processes, client experience, and business growth.
- Participate in occasional after-hours support or emergency coordination when required.
Skills
Required
- Minimum 2 years of experience in home care, community health, healthcare coordination, or a related field.
- Experience as a Care Coordinator, Home Care Scheduler, Client Care Coordinator, Case Manager, Home Care Supervisor, or a similar role.
- Strong knowledge of home care operations and client care planning.
- Excellent communication, interpersonal, customer service, and relationship-building skills.
- Experience developing referral sources and supporting business development initiatives.
- Strong organizational, leadership, problem-solving, and time-management skills.
- Ability to work independently while managing multiple priorities in a growing organization.
- Proficiency with Microsoft Office, Google Workspace, CRM software, or scheduling software.
- Valid BC Driver’s Licence and access to a reliable vehicle.
- Ability and willingness to travel throughout Surrey and surrounding communities.
- Ability to successfully complete a Criminal Record Check.
- Legally authorized to work in Canada.
Preferred
- Experience working with a private home care agency or community-based healthcare organization.
- Experience conducting client assessments and developing care plans.
- Experience recruiting, scheduling, supporting, and supervising caregivers.
- Knowledge of BC home care industry standards, WorkSafeBC procedures, and PIPA privacy requirements.
- Experience using home care management, scheduling, or CRM software is considered an asset.
