CAD $44.85 – $56.02 per hour.

Job Summary

Fraser Health seeks a dedicated clinician to provide care and support to individuals and families affected by mental illness and substance use disorders. The role involves advocacy, counselling, psycho‑education, community resourcing and care coordination within the local service area, supporting a trauma‑informed, recovery‑oriented approach.

Detailed Overview

In accordance with Fraser Health’s Vision, Mission and Values, the clinician collaborates with interdisciplinary teams to deliver consistent, timely treatment and rapid re‑engagement. Responsibilities include intake, screening, assessment, treatment planning, referrals, crisis intervention, outreach, family support and coordination with community resources and acute care facilities.

Responsibilities

- Develop and implement recovery‑oriented treatment plans; provide case consultation and follow‑up support; maintain communication with the Clinical Coordinator.
- Provide individual, group and family support using a variety of interviewing, supportive counselling and assessment methods; share workload with team to manage cases and provide crisis intervention and outreach.
- Support the intake process, assess client needs through phone interviews or other methods, gather client history, and determine priority of care and eligibility for services.
- Facilitate client utilization of community services and rehabilitative programs; research and liaise with providers; assist with housing arrangements; provide referrals to appropriate services.
- Advocate on behalf of clients; provide information about appeal processes and assist with completing forms.
- Arrange client admission to hospital when required, including involuntary admissions,



and coordinate transportation.
- Serve as a link between community mental health and substance use services and acute care facilities to facilitate discharge planning.
- Offer individual and group sessions, including psycho‑education or skill‑development groups; facilitate family and lived‑experience activities; educate staff, clients and families.
- Maintain client records, chart in PARIS or other systems, prepare progress notes and reports, and ensure compliance with freedom of information laws.
- Provide follow‑up care and referrals to other health professionals.
- Participate in quality improvement, risk management and clinical research activities; suggest initiatives to improve care coordination.
- Engage in ongoing education to maintain current practice and stay informed of legislation.
- Act as a member of local committees and advisory boards; inform regional staff and community groups of team functions.
- Perform other related duties as assigned.

Education and Experience

Bachelor's degree in Social Work, Clinical Counselling or another relevant health discipline; two years’ recent experience working with clients affected by mental illness and substance use disorders, or an equivalent combination of education, training and experience. Eligible for registration or membership with applicable professional association.



Valid Class V BC Driver’s License and access to a personal vehicle for business purposes.

- Maintain confidentiality in accordance with policies.
- Rapid learning of recent methodologies, ideas and concepts.
- Deliver efficient service while meeting government and health region policies.
- Knowledge of community resources.
- Comprehensive understanding of emergency triage, mental health assessment and counseling principles.
- Knowledge of psychopharmacology, crisis theory and de‑escalation techniques.
- Familiarity with DSM‑V Multiaxial Diagnosis or ICD classifications.
- Physical ability to carry out the duties of the position.
