GH-CCMH is also a member of the Thriving Minds Mental Health Access Collaborative with CAMH and SickKids and a core service provider in the Toronto sector of infant, child and youth mental health services funded by the Ministry of Health. GH-CCMH provides prevention, early intervention and health promotion services across the city, funded by MOH and other sources. The Centre offers a range of services from prevention and early intervention to counseling and therapy, assessment and intensive supports. Services are offered in person and in virtual format.We are currently seeking applicants for a skilled Child and Family Therapist for our Community Outreach & Intervention Initiative (COII). The COII program aims to address the needs of clients residing in three priority areas who do not seek or engage with traditional service models (cannot attend an agency site, do not have a safe space at home for virtual care) by enabling a pathway of service that travels to the client or builds access in a safe, appropriate workplace (e.G., community site, home).The successful candidate will work in a multi‑disciplinary team and have access to experts in the field, ongoing clinical supervision and support, as well as training and development opportunities. The Child and Family Therapist will provide direct mental health treatment services, including individual, family and/or group treatment to clients within the context of the family,



school and community.Key ResponsibilitiesWork collaboratively with external stakeholders in the community to engage with children, youth and families who otherwise may not connect with mental health servicesProvides individualized assessment, treatment and follow‑up servicesProvides single‑session therapy with the team as well as other brief services as neededProvides case management, liaison and referral services for clientsProvides virtual and telephone treatment as neededCoordinates services with internal and external service providers, requesting and reviewing existing documentation and reports to inform client service and treatment planningCollaborates with the family, the supervisor and inter‑professional teams in the development of a service plan and to support treatment planningFacilitates therapy groups for families with young childrenCompletes all client documentation (including, but not limited to, contact notes, service plans, treatment reports, incident/serious occurrence reports, referral forms, etc.) in a timely and succinct manner meeting agency, professional and legal requirementsEnsures that all required client tracking and statistical information is submitted according to Agency ProceduresPromotes the direction of the program and Centre by engaging in identifying, developing and implementing program goals, projects or servicesParticipates in relevant meetings, reports regularly, and as required to the designated supervisorContributes to the Centre’s academic mandate and to the field of children’s mental health through training,
