In this part time position, you will assess and address the nutrition and hydration needs of residents, ensuring compliance with regulatory standards.
Your role encompasses developing individualized nutrition plans, collaborating with other healthcare professionals, and providing essential dietary education for caregivers. You'll actively participate in quality improvements related to nutrition and hydration care across the facility. Key Responsibilities:
- Conduct nutritional assessments at admission and on significant changes
- Create and evaluate individualized care plans
- Document all residents' nutritional interventions
- Guide staff on dietary education and practices
- Assess and monitor high-risk nutritional residents Requirements:
- Degree in Food and Nutrition from an accredited university
- 1-2 years in gerontology or similar settings
- Membership in Dietitians of Canada
- Understanding of regulatory requirements and standards
