As a RAI Coordinator, you'll lead the organization and implementation of the Minimum Data Set for resident assessments. This role seeks a dedicated candidate with at least three years of experience in long-term care. Your focus will be on documentation accuracy, MDS training, and support for the interdisciplinary team, ensuring that our residents receive the best care.
Key Responsibilities:
• Coordinate the MDS implementation for the interdisciplinary team
• Provide education and software training using a train-the-trainer model
• Monitor MDS assessment completion and adherence to timelines
• Assist with coding assessments and ensure documentation accuracy
• Maintain an up-to-date resident assessment schedule
Requirements:
• Minimum three years experience in Long Term Care
• Knowledge of Long-Term Care Act and Ministry of Health standards
• Current RN or RPN registration in positive standing
• Understanding of MDS documentation requirements
• Strong project leadership skills
