This role is perfect for a nurse who enjoys consultative sales, community outreach, client advocacy, and relationship management while still using their clinical knowledge and compassion every day.What You’ll Be DoingAs our Client Care Manager (RPN/RN), you’ll be the face of our organization in the community and a trusted guide for families looking for support.Build strong, lasting relationships with clients, families, healthcare professionals, retirement residences, hospitals, hospices, and community partnersMeet with potential clients and families to understand their needs and create customized care solutionsConduct in-home Care Consultations and Quality Assurance visits dailyHelp families feel confident, supported, and cared for during emotional and important life transitionsDevelop referral relationships and maintain ongoing partnerships within the healthcare communityWork closely with our scheduling and caregiving teams to ensure exceptional client experiencesIdentify opportunities to improve client care plans and overall satisfactionBe a problem solver, advocate, communicator,
