Alberta Health Services is seeking a Licensed Practical Nurse to join Home Care in Sylvan Lake. The LPN will assess, plan, monitor and evaluate clients in collaboration with the Registered Nurse Case Manager and will coordinate and provide quality care in clients’ homes or alternate settings as determined by assessments.
The role involves working within an interprofessional team, applying nursing processes and patient- and family-centered care in alignment with standards and policies.
