Alberta Health Services is seeking an LPN for our Integrated Home Care team. You will work with an interprofessional health care team to provide direct nursing care across the life span, guided by CLPNA standards.
Under supervision of the RN Case Manager, you will assess, plan, implement and evaluate client and caregiver needs and contribute to secure, high-quality, patient- and family-centred care in a community setting.
