As an Integrated Support Worker, you will be part of a energetic inter-professional team, focusing on rehabilitation and support for individuals aged 16 and above.
Your role includes implementing individualized treatment plans, advocacy, and crisis intervention. Collaborating with clinical team members, you will teach life skills and support community integration through transport and appointment assistance. Key Responsibilities:
- Deliver individualized treatment and support plans
- Provide crisis intervention and advocacy for clients
- Collaborate with the clinical team on care plans
- Conduct life skills teaching sessions with clients
- Assist clients with transportation and community appointments Requirements:
- Diploma in health sciences or related fields
- Three years’ experience in mental health services
- Certification in First Aid and CPR
- Experience in community-based mental health settings
