Your role will encompass providing assessments, education, and behaviour modification support to a diverse range of patients. Working closely with fellow health professionals, your insights will guide interventions that foster wellness and empower individuals to make positive lifestyle choices. The position offers a chance to actively participate in quality improvement initiatives within a cooperative environment. Key Responsibilities:

- Perform initial assessments on patient referrals
- Assist with patient goal setting through behaviour strategies




- Educate clients and families on lifestyle modifications
- Develop group classes for psychoeducation
- Coordinate care with referrals to other services Requirements:
- Bachelor’s degree in relevant health field
- Appropriate professional registration in Alberta
- Proven communication and organizational skills
- Capacity to work flexibly within a team
