The Mental

Health & Justice Supportive Housing Case Manager coordinates rent supplement units for individuals aged 16 and older with mental health and justice involvement. The role seeks housing options, builds relationships with landlords and clients, and supports client transition to stable housing.

Qualifications - BSW preferred or equivalent experience - Knowledge of mental health issues, dual diagnosis and housing resources - Applied knowledge of community resources and agencies - Strong networking and outreach skills - Ability to assess energetic risk and provide side‑by‑side assistance in activities of daily living - High communication skills and supportive counselling expertise - Understanding and applied knowledge of Recovery Model, Brief Therapy, Strengths Perspective - Problem‑solving skills and ability to teach these skills to clients - Cultural competence for diverse populations - Ability to work independently and professionally - Computer literacy - Valid driver’s license Duties - Adhere to all CMHA policies and procedures - Outreach and collaborate with programs for individuals involved in the criminal justice system (e.g., mental health court, diversion programs, post‑custody enhancement)



- Collaborate with housing providers and landlords - Assess eligibility and develop individualized needs assessments for clients - Facilitate access to appropriate housing and coordinate use of rent subsidies - Develop personalized service plans in partnership with clients, case managers, and landlords - Conduct ongoing risk assessment and monitoring of housing stability - Advocate for clients, linking them with necessary supports and resources - Meet with clients regularly to review compliance and progress - Consult with landlords, families, and social service providers - Provide side‑by‑side support and supportive counselling - Model problem‑solving and priority‑setting skills for clients - Develop recovery plans focused on housing maintenance - Refer clients to community agencies and services as neededMaintain client records per CMHA policies - Participate in staff and administrative meetings, committees, and professional development opportunities - Ensure workplace health and safety compliance and client safety protocols - Support a safe environment and prevent adverse outcomes in service delivery - Maintain self‑care practices - Perform other duties as directed EEO Statement The Canadian Mental Health Association, Kenora Branch welcomes and encourages applications from people with disabilities.
