Position #: 9003275 Expected Start Date: August 10, 2026 Union: SUN Facility: Tisdale Hospital City/Town: Tisdale Department: Clinical Integration Chronic Disease Management Type: Casual FTE: 0 Relief: No Float: No Hours of Work: Field Hours Salary or Pay Band: Pay Band Nurse B $42.040 to $52.440 Travel Required: Yes : Assisting with developing, coordinating and conducting inpatient and outpatient programs and clinics; using established criteria to assess and plan each client's self-management care needs, providing intervention ensuring timely access to care; providing productive and effective individual self-management education/counseling for patients/families/caretakers in all aspect of management care and treatment. This includes liaising with families,
physicians, dietitians, staff nurses and community agencies.
