In this position, you'll deliver essential health services focused on mental health and substance use while working collaboratively within a multidisciplinary team.
You will drive positive health outcomes by providing primary care and advocating for client needs, all within a harm reduction framework. Engage with individuals facing systemic barriers and contribute to health promotion strategies in the community. Key Responsibilities:
- Assess and treat clients' health conditions
- Coordinate sexual health visits and screening clinics
- Advocate for clients in accessing health services
- Collaborate on program development and evaluation
- Educate clients about health and lifestyle issues Requirements:
- Current registration as Nurse Practitioner with CNO
- Minimum of 2 years clinical experience
- Evidence of controlled substances education
- Experience in primary care/community settings is beneficial
