You will work within an inter-professional team, ensuring accurate clinical assessments and care plans are continually updated. This role involves direct resident care, medication administration, and close collaboration with the Director of Care during resident transitions. Key Responsibilities:
- Provide patient-oriented nursing care and support
- Conduct clinical assessments and maintain care plans
- Document care compliance with standards and policies
- Assist with scheduling and facilitating resident care conferences
- Implement changes to care plans based on evaluations Requirements:
- Licensed Practical Nurse diploma and current registration
- 3+ years experience in seniors’ health
- Current CPR certification
- Proficient in Microsoft Office applications
