At Spectrum Health Care, our mission is to reimagine home care for clients and families by providing the products and services they need whenever care is required, delivered by the most qualified and compassionate staff.CompensationSalary for this role is $65,000 – $74,000 per year, based on experience.Job DescriptionTravel to patient’s homes in the communityWork collaboratively with multidisciplinary teams in a community settingAssess patient needs, according to the timeframe established by contractual agreements, formulate the plan of care and assess progress toward outcomesResponsible for the management of staff, conducting performance appraisals and implementing progressive disciplinary actionGather, record and analyze patient and employee feedback and maintain an awareness of key indicators such as caseload volume, staff turnover, occurrence rates, satisfaction levels, risk levels, and performance appraisal completion ratesEnsure that all patient concerns and complaints are addressed and documented in a timely mannerQualificationsCurrent registration with the Colleges of Nurses of Ontario (CNO)
