Empower residents at AgeCare Samac as a RAI Coordinator. In this pivotal role, you will oversee MDS administration and provide essential education to an interdisciplinary team. As a RAI Coordinator, you'll lead the organization and implementation of the Minimum Data Set for resident assessments.
This role seeks a dedicated candidate with at least three years of experience in long-term care. Your focus will be on documentation accuracy, MDS training, and support for the interdisciplinary team, ensuring that our residents receive the best care. Key Responsibilities:
- Coordinate the MDS implementation for the interdisciplinary team
- Provide education and software training using a train-the-trainer model
- Monitor MDS assessment completion and adherence to timelines
- Assist with coding assessments and ensure documentation accuracy
- Maintain an up-to-date resident assessment schedule Requirements:
- Minimum three years experience in Long Term Care
- Knowledge of Long-Term Care Act and Ministry of Health standards
- Current RN or RPN registration in positive standing
- Understanding of MDS documentation requirements
