You will focus on chronic disease management and preventative care, contributing to quality improvement and mentorship for learners. This role ensures you make impactful connections within diverse populations while enjoying a healthy work-life balance. Key Responsibilities:
Deliver evidence-based primary healthcare services
Support chronic disease management and health promotion
Participate in interprofessional case conferencing
Mentor and teach learners when applicable
Engage in quality improvement initiatives Requirements:
Licensed with CPSO for independent practice
Certification in Family Medicine preferred
Solid commitment to team-based care
Interest in community health and preventive care
