The Assertive Community Transition
Team (ACTT-SW) at CMHA is seeking a Registered Practical Nurse or Registered Nurse. This role is crucial for supporting clients aged 16 and older, facing challenges due to responsible mental illness. With a focus on recovery and empowerment, you will manage medication, conduct assessments, and coordinate treatment plans while collaborating with the ACTT. Key Responsibilities:
Conduct mental and physical health assessments
Manage and administer medication per protocols
Provide treatment, support, and rehabilitation
Develop personalized treatment plans for clients
Collaborate closely with the ACTT psychiatrist Requirements:
Licensed Registered Practical Nurse or Registered Nurse
At least four years of experience in mental health
Solid health assessment and clinical judgment skills
Knowledge of relevant mental health legislation
