Cota Health is seeking a dedicated Case Manager to support clients with dual diagnosis, including developmental disabilities and mental illness. You will develop goal plans, coordinate services, and provide coaching to help clients reach recovery objectives.
You will work with families and partners across health, justice and developmental services to ensure holistic support. A post-secondary degree with 2–3 years of related experience, or a BSW/MSW with 1+ year is required.
