Take the lead as a Supportive Housing Case Manager in Happy Valley-Goose Bay, NL. Deliver essential case management and life skills support for individuals facing complex needs. This full time, permanent position focuses on individuals at risk of homelessness in the Upper Lake Melville area.

You will employ an Intensive Case Management and Housing First Approach to provide assessments, counseling, and interventions. Collaboration with government services and multidisciplinary teams is vital to ensure comprehensive support for clients. Key Responsibilities:
Manage cases for individuals with complex needs in the community
Provide assessments, counseling, and life skills development
Coordinate services from various government and community agencies
Establish and monitor individual support plans for progress
Offer outreach and mobile crisis intervention for clients Requirements:
Bachelor’s degree in Social Work or related field
Minimum 2 years of case management experience
Current registration with NLASW
On-call availability and personal vehicle needed
