In this role, you will report to the Home Care Manager and collaborate with the Registered Nurse Case Manager.

Your nursing expertise will be essential in assessing, planning, and evaluating clients in their homes or alternate settings. You’ll contribute to a supportive interprofessional health care team while providing compassionate care and ensuring adherence to established standards and practices. Key Responsibilities:
Assess, plan, and evaluate client care in community settings
Provide direct nursing care to clients and families




Collaborate with health care team members for effective care
Maintain accurate records and comply with care standards
Support clients’ needs through critical thinking and problem-solving Requirements:
Completion of an accredited practical nurse program
Active registration with the College of Licensed Practical Nurses
Minimum two years of home care experience
Current Basic Cardiac Life Support certification
