Fraser Health Authority is seeking an experienced Licensed Practical Nurse to deliver community health care in a team-oriented interprofessional team. You will conduct client assessments, create care plans, and provide direct nursing care to clients in home and community settings.
The successful candidate will coordinate with primary care providers, families, and community services, support transitions, and promote health, safety and self-management for diverse populations across BC.
