North Bay Regional Health Centre is seeking a Registered Nurse to support the Hospital to Home Program – Mental Health (H2H MH). The nurse will stabilize after hospitalization and optimize recovery while linking clients to community supports. Care coordination and oversight will be tailored to each client’s goals.
The role requires knowledge of psychiatric conditions, psychotropic meds, and recovery-focused practices, with solid communication and teamwork in a multidisciplinary setting.
