As a vital part of the interdisciplinary Seniors Care Team, you will enhance seniors' independence while working directly under the Manager of Seniors Western Zone. Your responsibilities will include planning social work services, collaborating with community partners, and fostering emotional well-being for patients and families.
Key Responsibilities:
• Develop and implement social work services for seniors
• Collaborate with the interdisciplinary team in care delivery
• Engage with community partners to enhance service integration
• Provide clinical education for Social Work students
• Maintain adherence to professional standards and protocols
Requirements:
• Master's degree in Social Work required
• Minimum of two years clinical experience needed
• Certification with Nova Scotia Association of Social Workers
• Experience in Geriatric Care preferred
• Strong counseling and communication skills essential
