Direct Service Responsible for screening, assessment and provision of nursing interventions and supports to individuals in the community Collaborate with mental health and addiction service providers to support the mental health, addiction and primary care health needs of clients who may be in crisis or involved with community mental health and addiction agencies Supporting intake services Providing addiction medicine in the community Administer urine drug screens Use virtual technology to connect with individuals who experience barriers to system and treatment access Promote telemedicine to healthcare professionals, clients and the community Assist with program development, implementation and evaluation Qualifications – Required College diploma or university degree in nursing (primary proof of credentials will be required during the hiring process i.E. original degree/diploma or official transcript) Registered Nurse (RN) in good standing with the College of Nurses of Ontario (CNO) Current, satisfactory Police Vulnerable Sector Check (PVSC)

External applicants will start at Level 1 with grid progression based on seniority up to Level 8 ($41.41 per hour); for internal applicants, grid level placement will occur as per the Collective Agreement. Working hours: 70 hours over a two‑week period; Monday, Wednesday, Thursday, Friday, 8:30 am‑4:30 pm, and Tuesday evenings from 1:00 pm‑9:00 pm.
