Title: Youth SupportWorker Number of Positions: 1 Competition Number: C189 Location: PhoenixCenter (Main Campus) Position Status: PartTime, Permanent Shift Timings & Days: (Sunday, Monday) 08:00am - 04:00pm Reports to: ProgramManager Compensation: (Grid21) $29.76 - $31.64 Job Summary The primary role of a Youth Support Worker is to build trusting, supportiveprofessional relationships with youth and young adults. This role workswith a multidisciplinary team to support clients to help them reach their goalsand transition to healthy, confident adults as per a pre-established planof care. Working in the Youth & Young Adult (YAYA) Program, this rolewill provide youth and young adults with their basic and immediate needs,ensure their physical and emotional well-being and safety, support themwith relapse prevention and treatment, and help them to transition back tothe community when they may be ready.
Client Services Welcome and orient youth/young adults accessing Phoenix services upon entry to the Program (i.e., introduce andexplain services provided, facility tours, complete program orientation,etc.)
Actively engage with and develop relationships withat-risk youth and young adults 17-24 years of age
Coordinate youth meetings and activities, includingrecreation and life skills, individually and in consultation with theprimary treatment providers
Respond to and assist with the provision ofclients' immediate needs, including food, clothing,recreational opportunities, etc.
Assist clients with the completion of instrumentaldaily activities, including household upkeep andcommunity participation
As per pre-established care plans, workcollaboratively with other staff to assist youth in attaining shortand long-term goals, strategies for meeting these goals, and anaction plan for implementation, based on inputs provided by the MHSU team.
Assist youth with accessing external communitysupports and accompany them if needed
Respond to any medical emergencies or potentialbehavioral, mental health, emotional, or substance use concerns usingpre-established trauma-informed practice.
Maintain the cleanliness and safety of theresidence, including laundry as needed
Manage youth case files, including all relevantreporting and documentation




Attend all training and meetings as directed byyour manager
Complete room searches and room cleans upon clientdischarging from program
Monitor site security and respond to safetyconcerns, including access to/from the residence, keeping trackof clients' whereabouts, etc.
Complete critical incident reports as per policy
Other duties as required
Qualifications Grade 12
Certificate in Community Social Service orequivalent
Recent, related experience of one year providingcommunity social services to youth and young adults and working withyouth and young adult populations, substance use services, mental health,corrections, etc., or equivalent
Valid Class 5 driver's license with clean driver's abstract
Standard First Aid with CPR-C-AED certificatepreferred
Food Safe certificate preferred
Satisfactory completion of Nonviolent crisisintervention (NVCI)
Food Secure Certification is required
