Drive impactful youth health initiatives with Durham Community Health Centre as a Nurse Practitioner. This part time role focuses on delivering primary care and wellness services tailored to youth needs. As part of the Youth Wellness Hub team, you will be responsible for addressing the diverse health challenges faced by youth, including mental health and substance use issues.

Through trauma-responsive care, these services will support equitable access to health resources and engage youth in their care journey. Key Responsibilities:

- Conduct youth-centered assessments and treatments
- Manage pharmacotherapy for varied health concerns
- Collaborate with teams for integrated care solutions
- Promote health education and wellness initiatives
- Engage youth in their health decision-making processes Requirements:
- MSN with completed NP program and certification
- Minimum of 3 years in primary care roles
- Current CNO registration in Ontario as NP
- Knowledge of community health services preferred
