Position Summary

We are seeking a compassionate, organized, and patient-focused Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our family medicine practice. The successful candidate will provide high-quality nursing care to patients of all ages, support physicians during examinations and procedures, promote preventive healthcare, and contribute to the efficient operation of the clinic. The ideal candidate is a strong communicator who thrives in a collaborative, fast-paced outpatient environment.

Key ResponsibilitiesPatient Care

- Perform patient intake, including obtaining vital signs, medical history, allergies, and medication reconciliation.
- Assess patient concerns and communicate findings to the physician.
- Prepare patients for examinations and procedures.
- Administer medications, injections, immunizations, and treatments as prescribed.
- Perform wound care, dressing changes, and other nursing procedures within scope of practice.
- Monitor patient conditions and respond appropriately to urgent situations.

Clinical Support

- Assist physicians with minor procedures and examinations.
- Collect and process laboratory specimens.
- Perform point-of-care testing (e.g., urine dipstick, glucose testing, ECGs, spirometry) as applicable.
- Ensure examination rooms are stocked, clean, and prepared for patient care.
- Maintain infection prevention and control standards.

Patient Education

- Educate patients on medications, chronic disease management, preventive care, vaccinations, and healthy lifestyle practices.
- Provide follow-up instructions and reinforce physician treatment plans.
- Promote health screening and disease prevention initiatives.

Care Coordination

- Coordinate referrals, diagnostic testing, and follow-up appointments.
- Review laboratory and diagnostic results as directed by the physician.




- Communicate with pharmacies, specialists, hospitals, and community healthcare providers.
- Support continuity of care through effective patient follow-up.

Documentation

- Accurately document patient assessments, treatments, and communications in the Electronic Medical Record (EMR) Accuro.
- Maintain confidentiality in accordance with privacy legislation and professional standards.
- Complete required clinical documentation in a timely manner.

Quality & Safety

- Participate in quality improvement initiatives.
- Adhere to clinic policies, professional standards, and evidence-based practice.
- Maintain current knowledge of clinical guidelines and best practices.
- Participate in staff meetings and continuing education.

Qualifications Required

- Current registration and licensure as an LPN or RN in the applicable province/state.
- Current CPR/BLS certification.
- Experience in family medicine, primary care, ambulatory care, or community health preferred.
- Proficiency with Electronic Medical Records (EMR) in Accuro.
- Excellent communication, organizational, and interpersonal skills.
- Ability to work independently and collaboratively within a multidisciplinary team.
- Strong clinical assessment and critical thinking skills.

Preferred

- Experience with chronic disease management.
- Experience administering immunizations.
- Knowledge of preventive health guidelines.
- Experience with patient education and care coordination.

Job Type: Part-time

Pay: $30.00-$35.00 per hour
