Position Summary:
- The Canadian Mental Health Association, Simcoe County Branch seeks a Mental Health & Addiction Worker - Housing Support provides client‑centred housing support services in accordance with the agency’s policies, standards, and guidelines. The role involves working closely with individuals to assess housing needs, develop individualized service plans, and support clients in securing and maintaining safe, stable, and affordable housing. Through assessment, counselling, skills teaching, and ongoing support, the worker assists clients in accessing internal and external resources, building natural support networks, and working toward personal recovery and housing stability goals. The position may include outreach, aftercare, and follow‑up services; facilitation of groups as required; collaboration with community partners to ensure coordinated and culturally responsive care; and advocacy, harm reduction, and system navigation to reduce barriers to housing and support services.
What You'll Do:
- Conduct screening, intake, and ongoing assessments (e.g., SPDAT) to determine eligibility, strengths, needs, risks, and service priorities.
- Support client stability and well-being through monitoring needs such as substance use, mental and physical health, and risk, in alignment with program requirements.
- Collaborate with healthcare providers, including hospital and clinical teams, to support coordinated care and continuity of services.
- Facilitate transitions between institutional, custodial, and community settings through structured discharge and reintegration planning.
- Develop, implement, monitor, and update individualized service and housing plans in collaboration with clients and service teams.
- Provide direct, client‑centred support to individuals, families, and groups,
including counselling, case management, life‑skills development (e.g., activities of daily living, financial management), relapse prevention, goal‑focused support, and crisis intervention using harm‑reduction and recovery‑oriented approaches.
- Support clients in obtaining, transitioning to, and maintaining housing, including housing stabilization and tenancy‑related supports.
- Complete required documentation and data entry related to referrals, service delivery, and outcomes in designated databases (e.g., EMHware), in a timely and accurate manner.
- Provide advocacy and system navigation, reducing barriers and supporting access to housing, financial resources, services, and client rights.
- Facilitate access to internal and external services and supports, including referrals to CMHA SCB programs, community resources, and partner agencies to ensure continuity of care.
- Act as a primary liaison, as required, between clients, housing providers, and service teams to address concerns, support program implementation, and ensure quality of care.
- Provide timely crisis intervention and stabilization, coordinating responses with internal teams and external partners as needed.
- Support the maintenance of safe and supportive living environments, including completing unit or housing checks, supporting adherence to program guidelines, and addressing concerns in collaboration with housing providers.
- Assist with program coordination and oversight functions, as required, including supporting housing operations, contributing to inspections or program standards, and troubleshooting client, housing, or service issues.
- Build and maintain collaborative relationships with interdisciplinary teams, community partners, landlords, home operators, and/or referral sources to support coordinated and culturally responsive service delivery.
- Support the planning, implementation, and evaluation of programs and services, contributing to effective and responsive service delivery.
- Facilitate and coordinate access to services and supports, including discharge planning.
- Collaborate with management and team members to ensure effective, coordinated, and client‑centred service delivery, and participate actively in service planning, team meetings, supervision, committees, and agency‑wide initiatives.
- Comply with all agency policies, standards,
guidelines, and procedures, and contribute to their ongoing improvement.
- Represent the agency’s mission, values, and philosophy in all professional interactions.
- Provide transportation for clients within the community and, on occasion, outside the local area, in accordance with agency policies.
- Assist in the training, mentoring, and onboarding of new staff, students, and/or volunteers.
- Perform other related duties as assigned by the Program Manager or organizational leadership..
Required Qualifications:
- 1 - 3 years experience in the human services field.
- Must have one of the following:
- Human Service Worker, Social Service Worker, Addictions Diploma, or
- University degree in a human service related field, preferably Social Work.
- Experience in mental health and/or addictions (community-based setting preferred).
- Must have access to a vehicle to transport clients, a valid Ontario driver’s license, and adequate automobile insurance.
- Submission of a Vulnerable Sector Check (VSC), including a Criminal Record Check (CRC) in good standing.
- Fluency in French or another second language would be an asset.
- Legally entitled to work in Canada.
Salary Scale:
- RN: $61,600- $70,000 *per annum
Hours of Work:
- Thirty-five (35) hours per week, Monday to Friday
- Able to work within a flexible framework to respond to client needs.
