Care Coordinator (Full-Time)

Annapolis Valley Home Care is a locally owned and operated home care agency proudly serving communities throughout the Annapolis Valley. With a team of more than 100 Caregivers and a dedicated office staff, we are committed to providing compassionate, high-quality care to our clients while fostering a supportive workplace for our employees.

Due to continued growth, we are seeking a full-time Care Coordinator to join our office team.

Position Summary

The Care Coordinator plays a key role in the day-to-day operations of our agency by coordinating client services, supporting our Caregiver team, and ensuring exceptional customer service. This is a fast-paced, rewarding position that requires strong organizational skills, excellent communication, and the ability to manage multiple priorities.

Responsibilities

Responsibilities include, but are not limited to:

- Scheduling Caregivers and coordinating client services
- Conducting client consultations and home visits
- Building and maintaining positive relationships with clients and their families
- Coordinating Caregiver orientations and ongoing training workshops
- Assisting with recruitment, interviewing, hiring, and reference checks
- Maintaining accurate employee and client documentation
- Working collaboratively with the office team to ensure continuity of care
- Participating in the paid after-hours emergency phone rotation (approximately one evening per week and every fifth weekend)
- Providing exceptional customer service while responding to changing client and staffing needs

Qualifications

Preferred qualifications include:

- CCA Certification or equivalent education and/or relevant experience in healthcare or home care
- Previous scheduling or administrative experience




- Experience in human resources or recruitment is considered an asset
- Excellent verbal and written communication skills
- Strong organizational, time management, and problem-solving abilities
- Ability to multitask and work effectively in a fast-paced environment
- Proficiency with Microsoft Office and general computer applications
- Valid driver's license and reliable transportation
- Clean Criminal Record Check and Vulnerable Sector Check

Compensation & Benefits

- Full-time salaried position based on 75 hours bi-weekly
- Equivalent hourly salary of $25.00–$27.00, based on qualifications and experience

Schedule

Office Hours

Monday to Friday
8:00 a.m. – 4:00 p.m.

The workday includes one paid 15-minute break and one 30-minute lunch break.
