Position Summary

The Continuing Care Triage Pharmacist plays a key role in supporting residents in continuing care and supportive living sites by ensuring safe, timely, and well-coordinated medication management.This role focuses on reviewing incoming medication orders, triaging clinical issues, supporting LPN teams, and coordinating with prescribers, care teams, and pharmacy staff to ensure continuity of care.The Triage Pharmacist helps identify medication-related risks early, supports clinical decision-making, and ensures residents receive the right medications at the right time.

Key Responsibilities

1. Medication Order Triage & Clinical Review

- Review new prescriptions, medication changes, and refill requests from continuing care sites
- Prioritize urgent medication orders (antibiotics, controlled medications, OAT, Clozapine, insulin, anticoagulants, etc.)
- Assess appropriateness of therapy, dose, interactions, duplications, and allergies
- Clarify prescriptions with prescribers when needed
- Ensure timely processing of medication changes based on clinical urgency
- Support same-day or next-day implementation for high-risk medication adjustments when appropriate

2. Collaboration with Continuing Care Teams

- Work closely with LPNs, nurses, physicians, and site staff to ensure coordinated care
- Provide clinical guidance on medication administration questions
- Support medication adherence strategies
- Participate in care coordination discussions when needed
- Support integrated care model with pharmacists, LPNs, and other healthcare providers

3. Medication Profile Management

- Maintain accurate and up-to-date patient medication profiles in pharmacy software (e.g., Kroll)
- Review patient profiles for:
- therapeutic duplication
- drug interactions
- dosing appropriateness
- allergy conflicts




- Ensure medication records align with Netcare and other available clinical information

4. Risk Identification & Patient Safety

- Identify high-risk medications and residents requiring additional monitoring
- Support prevention of medication errors through proactive review
- Escalate concerns when clinically appropriate
- Support medication incident review and continuous quality improvement

5. Communication & Coordination

- Communicate with prescribers regarding medication clarifications and recommendations
- Coordinate with pharmacy fulfillment team to ensure timely medication preparation
- Support nursing teams with medication-related questions
- Ensure clear documentation of clinical decisions and communications

6. Support for Specialized Programs

- Support medication management for specialized programs such as:
- OAT (Opioid Agonist Therapy)
- Clozapine monitoring
- Complex chronic disease management
- Palliative care medication coordination
- Assist with medication reviews for residents when required

7. Documentation & Compliance

- Document clinical interventions and communications in pharmacy systems
- Follow regulatory standards and professional practice guidelines
- Maintain patient confidentiality and privacy standards
- Support compliance with continuing care medication management requirements

Qualifications

Education

- Bachelor of Pharmacy or Doctor of Pharmacy (PharmD)
- Licensed and in good standing with the College of Pharmacists

Experience





- Experience in continuing care, long-term care, or community pharmacy preferred
- Experience using pharmacy software (e.g., Kroll) considered an asset
- Experience working with interdisciplinary healthcare teams considered an asset

Job Types: Full-time, Part-time, Casual

Pay: $62.00-$65.00 per hour
