The Case Manager provides full-time support within Ryandale Transitional Housing’s residential program for individuals experiencing homelessness, mental health challenges, chronic illness, experiences of violence or trauma, and/or recovery from addictions. This role supports residents through trauma-informed case management, crisis prevention and intervention, life-skills development, housing stabilization, and community reintegration.
The Case Manager works independently and collaboratively within a service environment built on dignity, safety, empowerment, cultural respect, and recovery-oriented practice. Staff are expected to understand their own strengths and limitations and work within a team culture grounded in compassion, boundaries, accountability, and hope.
This position requires an understanding of trauma-informed care, harm reduction, and the unique barriers individuals in recovery from addiction face in accessing reliable housing and recovery supports.
Salary/Wage and Hours of Work
Rate of Pay: $23–$26 per hour (based on experience)
Schedule: Full-time; 35 hours per week
Shifts: Days and afternoons, with some evenings and weekends required.
Occasional on-call or overnight support may be requested
Responsibilities and Duties
1. Service Engagement
- Build strong rapport with residents using trauma-informed, strengths-based, and culturally sensitive approaches.
- Actively listen to resident concerns to support emotional regulation, self-awareness, and confidence-building.
- Provide ongoing coaching and support as residents work toward recovery and safe, sustainable housing.
- Facilitate in-house meetings, workshops, and supportive groups as appropriate.
- Provide crisis prevention and intervention, de-escalation, and safety planning as needed.
2. Assessment
- Conduct intake, orientation, and individualized needs assessments for all incoming residents.
- Monitor and document changes in residents’ physical, emotional, and situational wellbeing.
- Conduct regular reviews and updates of personalized support and safety plans.
- Communicate relevant information to Ryandale staff and community partners for effective care coordination.
- Assess environmental, social, and systemic factors that impact resident progress and safety.
- Maintain overall safety of residents, staff, the home environment, and the neighbourhood.
3. Service Delivery Education & Skills Development
- Support residents in building daily living skills, self-care, budgeting, meal planning, cleaning routines, communication, and conflict resolution.
- Encourage accountability, autonomy, and confidence-building.
Case Management
- Assist residents in setting personal goals and developing individualized recovery and housing plans.
- Advocate on behalf of residents with healthcare providers, housing partners, legal supports, and community organizations.
- Support residents transitioning from crises, shelters, correctional settings, treatment facilities, and unsafe living situations.
- Assist with discharge planning and long-term safety planning.
Outreach & Collaboration
- Participate in community meetings and collaborative networks to strengthen supports for Ryandale Residents
4. Organizational Capacity
- Participate in program development, evaluation, and documentation.
5. Organizational Responsibility
- Engage in regular team meetings, debriefing, supervision, and case reviews.
- Ensure accurate, thorough, and confidential documentation of resident interactions and case notes.
- Maintain professional boundaries, ethical behaviour, and resident confidentiality at all times.

Job Qualifications Knowledge, Skills, and Abilities
- Strong understanding of experiences related to trauma, violence, homelessness, mental health, and addictions.
- Ability to form supportive relationships while maintaining clear boundaries and emotional regulation.
- High emotional intelligence, empathy, and strong conflict-resolution skills.
- Experience with crisis intervention, safety planning, trauma-informed practice, and harm reduction.
- Strong organizational and administrative skills.
- Proficiency in Microsoft Office and willingness to learn additional programs (HIFIS/BNL).
Education
- Bachelor’s Degree or College Diploma in Social Work, Community Services, Addictions & Mental Health, Psychology, or a related field preferred.
- Equivalent combinations of training and experience will be considered.
Experience
- Two or more years of experience supporting individuals in shelters, transitional housing, mental health, addictions, or community-based services.
- Experience with case management and inter-agency collaboration.
- Lived experience is considered a strong asset.
Additional Requirements
- Acceptable CPIC (Criminal Record Check).
- First Aid/CPR certification preferred or willingness to obtain.
- Knowledge of WHMIS, fire safety, and health procedures preferred.
- Valid driver’s licence and access to transportation considered an asset.
