In collaborative consultation with Nursing Manager, multidisciplinary healthcare team members and cardiologists to provide seamless coordination of clinical planning, therapeutic activities and interventions for patients in Heart Health Value Stream. This may include participating in policy and procedure development, performing quality improvement and participating in education of nursing staff across area.
