Personal Support Worker (PSW)- BSO Lead
Maxville Manor is a 114-bed long term care home located in the village of Maxville, Ontario; just 50 minutes east of Ottawa or 35 minutes north of Cornwall.

We are seeking a Personal Support Worker with a specialty in BSO for a permanent full-time rotation. The PSW-BSO Lead provides leadership and support in delivering person-centered behavioural care to residents with responsive behaviours. This role works collaboratively with interdisciplinary teams to promote gentle, evidence-informed approaches, mentor PSWs in best practices, and support the development and implementation of individualized behavioural care strategies. The PSW-BSO Lead helps enhance team members' capacity through coaching, education, and role modelling while contributing to a safe, respectful, and therapeutic environment for residents, families, and team members.
Responsibilities and Duties:
Policy
- Complies with the code of ethics and standards of conduct as set out in the Maxville Manor Team Member Handbook.
- Upholds the Manor’s Mission, Vision, and Values.
- Complies with all provincial regulations and established department policies and procedures.
- Adhere to organizational policies, procedures, and best practice guidelines related to Behavioural Supports Ontario (BSO), resident care, privacy, infection prevention, and workplace safety.
- Support consistent application of behavioural care policies and gentle approaches among PSW team members.
- Participate in the review, development, and implementation of policies related to responsive behaviours, dementia care, and person-centred practices.
- Promote confidentiality and appropriate handling of resident information in accordance with legislative and organizational requirements.
- Uses mechanical lifts according to Zero Lift Policy.
- Supports residents with responsive behaviours using person-centered approaches.
- Implement and monitor individualized care plans.
- Complete behavioural tracking and documentation (including quarterly reports).
- Collaborate with clinical team members, families, and external partners.
- Promote a safe, respectful, and supportive environment for residents and staff.
Quality Improvement
- Contribute to quality improvement initiatives related to behavioural care, resident safety, and quality of life outcomes.
- Monitor trends in responsive behaviours, incidents, and care outcomes and communicate observations to leadership and the interdisciplinary team.
- Participate in audits, program evaluations, and improvement activities to enhance care delivery and team member practices.
- Encourage consistent documentation practices and evidence-informed care approaches among PSWs.
- Supports the continuous quality improvement of the department by providing constructive ideas and solutions to identified problems and challenges.
Health and Safety
- Promote a safe setting for residents, team members, and visitors by identifying, reporting, and mitigating safety risks.
- Support safe resident handling, mobility practices, and use of adaptive equipment.
- Participate in incident reporting, follow-up, and safety discussions related to responsive behaviours or workplace risks.
- Reinforce infection prevention and control practices and environmental safety standards.
- Act as a role model for respectful workplace behaviours and psychological safety.
- Complies with health and safety legislations and departmental safe job procedures
- Uses or wears protective equipment as required.
- Reports malfunctioning equipment or other safety issues to the supervisor.
- Adheres to strict hand hygiene practices.
Training
- Provide mentorship, coaching, and informal education to PSWs regarding behavioural support strategies, gentle approaches, and person-centred care.
- Assist with orientation of new team members to behavioural support practices, documentation standards, and responsive behaviour interventions.
- Support ongoing team member development through sharing best practices, case discussions, and role modelling effective communication strategies.
- Collaborate with leadership and external partners to identify learning needs related to behavioural care.
- Attends compulsory and in-service training as required to better meet the needs of residents.
- Completes all annual training requirements.
- Attends team member meetings as required (including Quality Team Meetings, Care Conferences).
- Assists with the coordination of the BSO Ambassador Program at Maxville Manor.
Clinical Responsibilities
- Support implementation and evaluation of individualized behavioural care plans in collaboration with the interdisciplinary team.
- Provide direct care and behavioural support interventions while modeling best practice approaches for team members.
- Observe, document, and communicate changes in residents’ cognitive, emotional, physical, or behavioural status.
- Facilitate effective communication between residents, families, PSWs, nursing team members, and BSO partners regarding behavioural care needs.
- Assist in developing strategies that promote resident independence, dignity, safety, and quality of life.
- Adheres to the plan of care as determined by the care team.
- Assists with resident personal care/hygiene, nutrition and activities of daily living.
- Observes and reports on changes in resident condition to supervisor.
- Supports residents with cognitive/psychological deficits by utilizing care interventions to minimize potential for responsive behaviours.
- Assists with ambulation, mobilization, transferring, and adaptive aids.
- Demonstrates understanding of resident needs and maximizes abilities.
- Maintains resident information confidential.
- Refers resident or family concerns to the nurse in charge of unit.
- Notifies supervisor of any concerns impacting resident safety.
- Assists in maintaining a protected and clean workplace, including resident equipment, belongings, and nursing equipment.
- Completes documentation in PointClickCare in POC and notes, as well as E-DOS Mapping.
- Completes all other tasks as assigned.
Qualifications:
- Personal Support Worker Diploma from a recognized Community College
- Completion of the BSO Foundation Course (required)
- GPA training (required)
- P.I.E.C.E.S training considered an asset
- Dementia Studies certificate (asset)
- Experience in Long Term Care setting (asset)
- Knowledge of quarterly BSO tracking and reporting processes
- Familiarity with DOS (Dementia Observation System) mapping
- Experience with PointClickCare (PCC) electronic documentation system
- Ability to de-escalate challenging behaviours
- Ability to deliver educational sessions on dementia care and responsive behaviours
- Mentoring and knowledge-sharing skills
