Overview
Join our dynamic healthcare team as a Pharmacist, where you will play a vital role in ensuring secure, effective, and personalized medication management for diverse patient populations. The successful candidate will provide safe, high-quality pharmaceutical care to patients receiving opioid agonist therapy (OAT), including methadone and buprenorphine/naloxone.In this energetic and impactful position, you will leverage your expertise to provide exceptional patient care, medication counseling, and collaborative support to healthcare providers. Duties
- Dispense methadone, buprenorphine/naloxone (Suboxone), and other prescribed medications accurately and safely.
- Prepare and administer witnessed methadone and buprenorphine doses in accordance with applicable regulations and skilled standards.
- Verify prescriptions for accuracy, appropriateness, dosage, interactions, allergies, and contraindications.
- Counsel patients on medication use, potential side effects, adherence, storage, and safety.
- Monitor patient medication therapy and communicate concerns or recommendations to prescribers.
- Maintain accurate patient records and controlled substance documentation.
- Manage inventory and security of narcotics and controlled substances.
- Perform narcotic counts, reconciliation, and required record keeping.
- Ensure compliance with provincial College of Pharmacists standards, federal controlled substances regulations, and pharmacy policies.
- Provide naloxone education and supply naloxone kits where applicable.
- Support harm reduction initiatives and promote safe medication practices.
- Collaborate with physicians, addiction specialists, nurses, and other healthcare professionals to optimize patient care.
- Supervise pharmacy technicians and pharmacy assistants.
- Maintain a clean, organized, and professional pharmacy environment.
- Participate in quality improvement activities and continuing professional education.
- Proven experience working in hospital or acute care settings with exposure to pediatric and adult patient populations
- Strong knowledge of medical terminology and medication administration procedures
- Excellent patient care skills with a compassionate approach to diverse healthcare needs
- Ability to interpret prescriptions accurately and ensure safe dispensing practices
- Effective communication skills for collaborating with multidisciplinary teams and educating patients
- Familiarity with pharmacy software systems and inventory management tools
- Commitment to ongoing professional development within pharmacy practice standards
Pay: $42.11-$67.39 per hour
