Generations Medical Centre (GMC) is a not-for-profit Family Health Organization (FHO) serving Thorncliffe Park and Flemingdon Park — designated City of Toronto Neighbourhood Improvement Areas. Through our Interprofessional Primary Care Team (IPCT) model, GMC integrates physicians, nurse practitioners, nurses, social workers, dietitians, and allied health professionals to provide comprehensive, equitable, and patient-centred primary care.

GMC is seeking a compassionate Registered Social Worker to join our interdisciplinary team in providing brief counselling, psychosocial assessments, mental health support, and system navigation for a diverse patient population.

The Role

- The role includes supporting patients experiencing anxiety, depression, trauma, grief, addictions, and life transitions through evidence-based approaches such as CBT, motivational interviewing, and solution-focused therapy.
- The Social Worker will also assist patients with social determinants of health needs, including housing, income supports, ODSP/OW, settlement services, newcomer and refugee supports, IFHP navigation, and connection to community resources.




- The role involves close collaboration with physicians and allied health providers, participation in case conferences, care planning, quality improvement initiatives, and maintenance of timely EMR documentation.

Qualifications

- The successful candidate must hold a BSW or MSW from a recognized institution and maintain active registration in valuable standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
- Required qualifications include experience in primary care, community health, or mental health settings; proficiency in brief counselling and trauma-informed practice; strong knowledge of Ontario social services and community resources; and a commitment to serving diverse and underserved populations.
- Experience within an FHT, FHO, or IPCT model, newcomer health, additional language skills, EMR experience, and group facilitation are considered assets.

Pay: $70,000.00-$75,000.00 per year Work Location: Hybrid remote in North York, ON
