Position Summary The Resident Care Coordinator is responsible for coordinating, overseeing, and supporting the delivery of individualized resident care at Spruce Bough Supported Living. Working collaboratively with residents, staff, healthcare providers, community partners, and leadership, this position ensures residents receive the supports required to maintain their health, wellness, safety, autonomy, connection, and quality of life.

Spruce Bough provides supported living for individuals with complex health, mental health, substance use, and other support needs. The Resident Care Coordinator must understand and support a harm reduction approach and have experience and comfort working with individuals who may be actively using substances and who experience complex medical, mental health, and behavioural health needs.

This position oversees resident referrals, screening, assessments, intake, onboarding, and the development of individualized care plans. Care plans guide the work of the Resident Care Coordinator and direct the approach of the broader support team to ensure supports are coordinated, consistent, and focused on resident goals, quality of life, health, wellness, and connection.

The Resident Care Coordinator provides complex care coordination, advocacy, and daily operational direction to the Spruce Bough care team, including Resident Support Workers, Personal Support Workers, and the Service Coordinator. This position ensures staff have the guidance, training, and support required to effectively implement resident care plans and provide high-quality, resident-centered supports.

Key Responsibilities

Resident Care Coordination and Planning

- Oversee resident referrals, screening, assessments, intake, and onboarding processes.
- Develop, implement, and regularly review individualized resident care plans in collaboration with residents, staff, healthcare providers, and community partners.
- Ensure care plans reflect resident goals, strengths, needs, choices, risks, and required supports.
- Coordinate and oversee resident health, wellness, mental health, substance use, and daily living supports.
- Monitor resident progress, changing needs, and effectiveness of supports, updating care plans as required.




- Advocate with and for residents to support access to healthcare, services, and community resources.
- Support residents in making informed choices while ensuring appropriate supports are in place to promote safety and wellness.

Health and Wellness Coordination

- Coordinate healthcare, mental health, wellness, and community supports based on resident needs and preferences.
- Coordinate medical appointments, transportation, accompaniment, follow-up care, and communication with healthcare providers.
- Support hospital admissions, discharge planning, and transitions in care.
- Coordinate medication management processes, including staff training, competency development, documentation, and compliance monitoring.
- Act as a primary point of contact with pharmacies and healthcare providers regarding resident health needs.
- Support infection prevention and control practices, emergency preparedness, and health and safety initiatives.

Team Leadership and Operational Support

- Provide daily operational direction, coaching, and support to Resident Support Workers, Personal Support Workers, and the Service Coordinator.
- Ensure staff understand and implement resident care plans, behavioural support plans, and individualized supports.
- Provide coaching, mentoring, and training to strengthen staff skills in supporting residents with complex needs.
- Monitor implementation of care plans and ensure resident supports are consistent, coordinated, and documented.
- Work collaboratively with the Spruce Bough Manager and leadership team to identify program needs, risks, and opportunities for improvement.
- Support consistent implementation of organizational policies, procedures, documentation standards, and best practices.

Documentation, Quality Improvement and Collaboration





- Ensure resident documentation accurately reflects resident needs, goals, supports provided, progress, and changes in circumstances.
- Monitor documentation standards, including APRICOT requirements, and act as an APRICOT Super User.
- Support quality improvement initiatives, incident reviews, risk management, and service improvements.
- Review trends and identify opportunities to improve resident care, safety, and quality of life.
- Maintain cooperative relationships with residents, healthcare providers, pharmacies, hospitals, community agencies, and service providers.

Qualifications

- Diploma or degree in Nursing, Social Work, Human Services, Health Care, Community Services, or a related field. Equivalent combinations of education, training, and experience may be considered.
- Experience providing complex care coordination, case management, or individualized supports to people with complex needs.
- Experience supporting individuals with active substance use, mental health challenges, chronic health conditions, and significant support needs.
- Experience providing leadership, coaching, and operational direction to frontline teams.
- Experience working in supportive living, housing, homelessness, healthcare, Indigenous organizations, or social services is an asset.
- Knowledge of harm reduction, trauma-informed practice, culturally safe approaches, and person-centered care.
- Experience with medication management systems and healthcare coordination is an asset.
- Recent Vulnerable Sector Check is required.
- Valid Class 5 driver's licence is required.

Working Conditions

- This position includes a combination of day and evening shifts to provide overlap and continuity with the second care team.
- Work is primarily based at Spruce Bough Supported Living and includes regular interaction with residents, staff, healthcare providers, and community partners.
- Flexibility is required to respond to resident needs, urgent situations, and operational priorities.

Pay: From $40.00 per hour
