COMPANY OVERVIEW
Our diverse range of services includes workplace training and career services, community volunteerism, IT training, Life Coaching, Speech courses, supported recreation, and residential services. Our clients include people from all walks of life and experience, looking to move ahead in employment, personal goals, or who just want to make connections and volunteer in their community. Our programs and services are open to people 16 years of age and older living with a disability, mental health needs, FASD, chronic illness, brain injury, or sensory impairment.
We offer a unique range of options for individuals who may require support or transitional services.
This required schedule will be every second weekend, 8 hours, Saturday and Sunday
- Participates with the individual and the individual’s support network as appropriate to facilitate and support the development and documentation of individualized, person-centered plans to provide a framework for ongoing support and assistance. In general terms, the plans will focus on individual development, community disability/development and/or behavior management. This includes following the individual’s leads to accommodate and support their personal goals and choices to the extent possible.
- Facilitates, in more complex situations, without full supervisory accountability and authority, the implementation of support plans and facilitating intra-group communication.
- Assists, supports and encourages individuals to exercise their right and responsibility to make life choices and participate actively in meaningful, respectful and authentic relationships that align with and support their goals and aspirations.
- Being aware of the individual’s physical, social and recreational environments, and taking action to minimize risks and maximize participation/inclusion; and, when it is appropriate, requesting assistance and/or referring to more experienced resources.
- Being aware of and adhering to established policies, practices and operational protocols; this includes being aware of and adhering to ethical standards and guidelines, and medical protocols and standing orders relative to medication and other remedies.
- Assists, supports and encourages individuals to advocate on their own behalf, when appropriate and necessary, to have their needs met; and, when appropriate, assisting and supporting the individual’s desires, aspirations and life choices through interaction and advocacy within the individual’s social network.
- Provides holistic supports, in a person centered manner, that includes supporting each person’s right to explore and be educated about sexuality,
and to subsequently enjoy a lifestyle unique to their values.
- Assists, supports and encourages individuals to achieve their personal goals through the development of basic living and related, with a view to enhancing the individual’s confidence, competence and participation within their social network.
- Develops and implements marketing approaches for employment, recreational and other social interaction opportunities, with a view to creating a broader spectrum of options and natural supports within the community.
- Acts as a social, recreational and/or employment role model and coach to assist the individual to participate and contribute effectively with the community and/or in an employment or volunteer setting.
- Assists employers and others in the individual’s social network to create and maintain a safe and open environment for maximum participation.
- Maintains clear and objective records, and completing required reports and other documentation.
- Communicates in a professional, timely, accurate and respectful manner with individuals, families, guardians, staff and community representatives.
- Works effectively in a cooperative, team-oriented environment.
- Undertakes special tasks, projects and/or other assignments, including representing the agency on provincial committees.
- Supported daily living responsibilities include problem solving, life skills, emotional care, etc.
- Medication administration, including G-tube feeding.
- Cooking a variety of nutritious meals and assisting with feeding.
- Personal care, including bathing, showering, shaving, toileting, etc.
- Household cleaning, including but not limited to, sweeping, mopping, vacuuming, bathrooms, dusting, and sanitization.
Additional Expectations
- Participates alongside the individuals being supported. Taking part in activities such as swimming, bowling, and outdoor activities inclement weather.
- Attend team, agency and committee meetings as schedule permits.
- Promotes individual empowerment and independence by offering choices, teaching self-advocacy, and problem solving skills. Update and maintain knowledge regarding community resources and activities.
- Establish and maintain effective communication with clients, co-workers, guardians, Community Service Coordinators,
residential agencies, volunteer placements, and all stakeholders.
- Provide emotional support to individuals to assist them in expressing their feelings, desires, and frustrations.
- Maintain high ethical and professional standards and practices at all times.
- Be an active team member and remain open to suggestions. Empower team members through positive feedback and utilizing strengths and expertise. Participate in the facilitation of change, transition, and conflict resolution with individuals and team.
- Perform other duties as assigned related to the daily operation of Cosmos Community Support Services Ltd. This may include, but not be limited to, working in other departments including the depots to assist in the delivery of quality service.
Education, Experience and Qualifications:
- Post-secondary education in Human Services or in a related field.
- Minimum of 2 years` experience in a related field.
The preferred qualifications include the following:
- In addition to the basic qualifications required, a Diploma or equivalent formal education in a relevant discipline; preferably, augmented by specialized training and progressively more responsible experience in a relevant discipline/setting.
- A practical, working understanding of relevant general and organization specific policies, procedures, operational protocols, legislation, professional codes of ethics and guidelines, contract terms and conditions, and other practice guidelines.
- A general understanding of the broader range of community resources available to support the individual’s participation and inclusion, and an understanding of how and when they may be accessed; and, the ability to interact effectively with others in the individual’s social network.
- An understanding of basic marketing and sales techniques as they may apply in the development of community opportunities and options for individuals being served.

- First Aid and CPR certification Preferred
- Nonviolent Physical Crisis Intervention Training Preferred
Work Ethic Expectations:
- Ability to effectively establish and maintain cooperative working relationships within a diverse multicultural environment.
- Occasional work outside business hours.
- Ability to work under minimal supervision.
